UToledo students and staff who plan to park in the Reserved Patient Areas.

PARKING IN RESERVED PATIENT AREA FORM

This form is for UToledo students, staff, and affiliates to voluntarily identify themselves if they plan to park in the Reserved Patient/Patient Visitor areas at UTMC (Green Lined Spaces). Submitting this form will alert ParkUToledo staff to not ticket a permit holder parked in the reserved patient areas.

*This form does not guarantee the submitter will not receive a ticket
*ParkUToldeo will investigate suspected abuse of this form with the assistance of UTMC leadership.
